If you are reading this, you have an upcoming appointment with one of our physicians and have received an invitation email for the TUCC Portal:

The login screen is on the top, Left of the screen
User your login (email address) / password from your email to complete the registration process and view your chart
You will be asked to verify your Date of Birth and gender to confirm your identity
